The Ohio Department of Health is celebrating an important milestone. The number of Patient-Centered Medical Home (PCMH) primary care practices in Ohio has topped 500. In August of 2014 there were 513 recognized PCMH practices in the Buckeye State. In Greater Cincinnati, including Northern Kentucky there are more than 165 PCMH recognized practices. You can find out which practices in your neighborhood are PCMH recognized but going to YourHealthMatters and checking PCMH when you do your search.
