On Tuesday, February 9, 2016, The Health Collaborative hosted a webinar about Greater Cincinnati’s readiness to pursue a CMS funding opportunity – Accountable Health Communities (AHC). The Health Collaborative is applying to CMS to serve as the bridge organization. This funding opportunity requires participation from key partners representing state agencies, clinical delivery sites, and community service providers. On this webinar we have provided an overview of the process and timeline of our application. Other key objectives include:
- To promote to stakeholders the value of the AHC opportunity to the community and inform interested parties of our intention to pursue this opportunity as the bridge organization.
- To share the expectations and requirements of the AHC program with stakeholders.
- To explain our process for designing our approach, aligning partners, and drafting our proposal.
- To solicit feedback from stakeholders.
