Practice Facilitator: The Practice Facilitator is responsible for the practical delivery of quality and process improvement methodologies to practice teams.

Quality Improvement Coordinator: The Quality Improvement Coordinator is responsible for day-to-day support of practice transformation and quality improvement initiatives.
