
Sharing information across systems of care is a universal challenge facing behavioral health providers. This challenge affects providers, community partners, and patients. This leads to ineffective care, gaps in service, and outdated medical charts.
The Health Collaborative is now working with Cincinnati Children’s (funded by The Millstone Grant) to scale the first of its kind behavioral health patient attribution system. This tool is designed to create more cross-system visibility into treatment relationships allowing for more access to clinically-actionable patient data.
