The Health Collaborative Awarded Grant from the Fifth Third Foundation

CINCINNATI, OH (April 25, 2022) —The Health Collaborative (THC) has been selected to receive funding from the Foundation Office at Fifth Third Bank* through the Ohio Valley Foundation, to support the improvement of health equity through transformational investments in technology.

This grant will help improve individual and population health by bringing together data to ensure that providers have full access to patient health records.

THC will employ the funding to develop and implement a more sustainable and scalable regional health technology solution that can be integrated with existing Electronic Health Record (EHR) software and used across health systems, community organizations, and other social service agencies.

Over the past 5 years, THC has participated in the Accountable Health Communities (AHC) program, funded through the Center for Medicare and Medicaid Services (CMS). With the AHC program ending, this award will help develop a sustainable technology solution that will help health systems with screening and assisting community members beyond the AHC project end date.

“The Health Collaborative has participated in the Accountable Health Communities (AHC) program for the past 5 years and although that program is coming to a close, we have recognized there is significant need to continue screening community members for health-related social needs,” stated Dawn Day, Senior Director, Population Health at THC. “We plan to collaborate with health systems and community-based organizations to identify need and build upon existing data solutions to implement a technology solution that will help address gaps in the current workflows.”

The Population Health team works to align the region on common, actionable priorities to advance health equity and outcomes, by addressing Social Determinants of Health identified through the Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP).
