The Health Collaborative provides a secure repository of patient health information as a business associate of healthcare providers/owners (“Data”). The Health Collaborative has developed processes and procedures that ensure the secure storage and appropriate uses of and access to the Data in compliance with all federal and state laws, including, among others, the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), the HITECH Act, and the ONC Information Blocking Rules.

Patient Access Requests

The Health Collaborative is unable to provide patients or their personal representatives with direct access to their Electronic Health Records. The Health Collaborative does not have the necessary technological resources to meet certain standards of security and privacy that are required under HIPAA for such a disclosure. Therefore, The Health Collaborative recommends that you contact your hospital, physician, or other healthcare provider in order to obtain a copy of your records directly from the source. You can click on the appropriate link below for contact information for the Medical Records Departments of our local health systems.

Third Party Requests

All third-party requests for Data will be handled in accordance with The Health Collaborative’s board approved Data Governance Policy and Process, a detailed summary of which is linked below. Please be aware that it will take some time to process your request. The Health Collaborative makes every effort to handle requests expeditiously, but because of privacy concerns, it is necessary to carefully consider each request. To begin the process, please complete the attached Data Request Form linked below and return it via email to datagov@healthcollab.org. Please do not hesitate to use this email address if you have any questions or need assistance with completing the Data Request form. Once we receive your form, we will be in touch with you as soon as possible.
